This is to notify you that a Consumer Report and/or Investigative Consumer Report will be conducted on you for employment purposes.

By submitting this application, I hereby authorize Gerber Poultry, Inc., & Gerber Farm Division, Inc. any and all corporations, former employers, credit agencies, educational institutions, law enforcement agencies, city, state, county, and federal courts, military services to release information about my background including, but not limited to, information about employment, education, consumer credit history, driving record, criminal record and general public records history to Gerber Poultry, Inc., & Gerber Farm Division, Inc.

I also consent to Gerber Poultry, Inc., & Gerber Farm Division, Inc. procuring or causing to be procured an Investigative Consumer Credit Report. If an adverse employment decision is made due totally or partially to the information on the credit report, Gerber Poultry, Inc., & Gerber Farm Division, Inc. will give me a copy of the credit report, a summary of my rights under the Fair Credit Reporting Act, and the source of the credit report so that I may contact them, if I wish. I release from all liability all persons, companies, schools supplying such information. I indemnify Gerber Poultry, Inc., & Gerber Farm Division, Inc. against any liability, which may result from making such requests. This release shall remain in effect for the length of my employment.

If employed, I further authorize Gerber Poultry, Inc., & Gerber Farm Division, Inc. to check my credit record, as needed, on a continuing basis as it relates to my employment. I am aware that I have a right to a written request within a reasonable time to receive additional detailed information as to the nature and scope of the investigation. I also waive my right to receive from a former employer a written notice disclosing any disciplinary action in my former employment.

I understand that you rely upon the accuracy of the information furnished by me, and that any misleading or incorrect statements made, intentionally or otherwise, as part of this application may be considered cause for rejection or dismissal. I understand that submission of an application does not guarantee employment.

In consideration of my employment, I agree to conform to the rules and regulations of Gerber Poultry, Inc., & Gerber Farm Division, Inc., and my employment and compensation can be terminated, with or without cause, and with or without notice, at any time at the option of either the Company or myself.
